I am submitting this letter as a formal complaint regarding my care under an infectious disease specialist, specifically concerning the refusal to consider or meaningfully evaluate the possibility of chronic Bartonella infection. While I understand that there is clinical debate surrounding chronic presentations of certain infections, I was concerned that my case was approached with a rigid framework that did not allow for discussion of chronic disease mechanisms, disease progression, or diagnostic limitations. Negative PCR testing was treated as definitive, without acknowledgement of its reduced sensitivity outside of acute infection or consideration of clinical history and long-standing symptoms. At no point was there a forward-looking discussion regarding how chronic or persistent infection could present differently from acute disease, nor were alternative diagnostic strategies or management pathways explained. The refusal to consider chronic disease was not accompanied by documentation, evidence-based rationale, or a plan for monitoring, reassessment, or referral elsewhere. As a result, I was left without treatment, explanation, or a transition-of-care plan despite ongoing and worsening symptoms. Communication during this time was inconsistent, and when it became clear that treatment would not be pursued, no formal discharge or guidance was provided. My concern is not a disagreement over diagnosis alone, but the absence of clinical reasoning applied to chronic disease, the lack of discussion around diagnostic uncertainty, and the failure to provide continuity of care when treatment was declined.
