Dr Mo Hafez took 6 months to send my mother's Plan G forms out. He was instructed many times and was very rude to her. He tried charging her extra after 6 months and multiple calls to send it again. Ended up having a clinic send the form. Ridiculous that he doesn't know basic protocols like Plan G for low income patients and would drag it on for 6 months just to bank the extra visits.
